• All surgery patients are required to have a pre-op physical by their primary care physician prior to surgery. After scheduling your surgery, please call your primary care clinic to schedule your pre-op physical.
• There will be eating/drinking guidelines that must be followed before surgery.  Specific instruction will be provided by the hospital/ambulatory surgery center based on the surgery time.
• Please refrain from using perfume, cologne or lotions the day of the procedure.
• We recommend that you dress comfortably, in casual, loose-fitting clothing and flat shoes.
• Please do not wear makeup or nail polish.
• Please remove all jewelry and piercings and leave your valuables at home. Bring cases for your glasses, contact lenses and hearing aids, if applicable.
• Please bring your insurance cards and photo identification (e.g., driver’s license or government-issued identification card).
• You will be required to have a responsible adult drive you home. All patients receiving sedation or anesthesia are required to have someone stay with them for the first 24 hours following your surgical procedure. You should not be alone.

There are separate charges for care or services associated with each surgery. This means you may receive more than one bill for your surgery from different companies providing different services. These may include:

• Hospital or Ambulatory Surgery Center – surgical facility (operating room)
• Surgeon’s office – for your doctor(s) and assist
• Anesthesia group – for anesthesia, if required
• Primary Care Clinic – for your history and physical exam (pre-surgery)
• Radiology – for any imaging, if required
• Laboratory – for laboratory tests run on samples (specimens) removed during a biopsy or surgery
• Pathology – for tissue specimens removed during surgery requiring further examination, if required

You will be billed for any balance due after your insurance company has paid your claim.
